Purpose

The Community of Hope Health Clinic is a medical mission which sees Shelby County adults who are at or below 200% of the Federal Poverty Guidelines and do not have access to Medicare, Medicaid, or VA Benefits.

History

Opened in 2008, the Community of Hope Health Clinic was formed with a Memorandum of Understanding with the Shelby County Health Department for use of its facility on Thursday evenings. 

 

In 2010, the number of patients constantly grew and CHHC changed its model in 2012 to become an appointment-based primary care clinic.

 

In 2014, CHHC began seeing patients on Mondays, Tuesdays, and Thursdays.​In May 2015, the Community of Hope Health Clinic Board of Directors hired the third Executive Director in the history of the clinic, Justin R. Johnston. CHHC further expanded its staff in 2016 with the addition of an office manager and then again in 2021 with the addition of Operations Director.​Since the start of the COVID-19 pandemic and shutdown, CHHC has administered thousands of tests and vaccines, started a physical therapy clinic, began treating patients for Mental Health through both medicine and counseling.

 

In October 2020, CHHC moved from the ADPH building in Shelby County to 2692 Pelham Parkway Ste E Pelham, AL 35124 right next to Pee Wee's Pawn Shop on Highway 31. Due to space concerns, the Community of Hope Health Clinic moved to 1970 Chandalar Office Park Circle Pelham, AL 35124 in July 2023.
